Koze Head Spa & More - Booking Policy

At Koze Head Spa & More, our mission is to provide every guest with a safe, relaxing, and professional experience. To protect the wellbeing of our clients and staff, we ask that you please review and acknowledge the following policies before booking or receiving services.

Appointment Booking & Deposits

  • Online Self-Booking

    • $30 deposit for services priced at $60 or more.

    • 50% deposit for services priced at $60 or less.

    • $100 deposit for all Permanent Makeup (PMU) services.

    • Deposits are non-refundable and required at the time of booking.

    • Group Bookings

    • Groups may book directly through our website or by calling 952-236-0265.

    • We require the full name and phone number of each guest so that intake and consent forms can be completed in advance.

    • Minors under 18 require a legal guardian present to sign a waiver.

  • Phone Booking with Payment Link

    • If booking by phone, a secure payment link will be sent.

    • The deposit link expires 24 hours after being generated.

    • Deposit is 20% of the total service cost regardless of price.

  • Booking for a Minor

    • For under 18 years old.

    • A guardian must be present during the appointment and sign the waiver.

Health & Safety Guidelines

Your health and the health of our team are our top priorities. Please reschedule or postpone your appointment if:

  • You have lice, contagious conditions, or communicable diseases.

  • You are experiencing cold or flu-like symptoms, fever, skin rash, or other contagious symptoms.

  • You have been exposed to someone with a confirmed contagious illness within the last 5 days.

  • You are part of a high-risk or vulnerable population and are concerned about exposure.

→ For the safety of all, we reserve the right to refuse service if these conditions are present.

Right to Refuse Service

  • Services may be declined or stopped if a client:

    • Displays inappropriate behavior or disrespects policies.

    • Makes requests that conflict with our protocols and does not agree to pay the difference for additional time, product, or service.

    • Brings unsafe, expired, or unapproved outside products.

    • Causes an employee to feel uncomfortable or unsafe.

→ Our staff and management fully support the right to refuse or discontinue service if a client’s actions, health, or requests compromise our policies, protocols, or employee wellbeing.

Outside Product Use & Consent

  • Outside products are generally not permitted.

  • If approved, a signed consent & liability release form is required.

  • Koze Head Spa & More is not responsible for undesired results, reactions, or complications caused by personal products.

Appointment Cancellations & Deposits

  • 48+ hours’ notice required for cancellations or reschedules.

  • Less than 48 hours’ notice = 50% fee of scheduled service price.

  • Cancellations must be made by phone at 952-236-0265.

  • Deposits are non-refundable, but with proper notice may be applied as credit toward a future service.

No-Show Policy

  • Missed appointments without notice will be charged 50% of the total scheduled service price.

  • Repeated no-shows may require higher deposits or full pre-payment for future bookings.

Late Arrivals

  • We kindly ask all clients to arrive on time so we can provide you with the full service you reserved.

  • More than 15 minutes late without notice will be considered a no-show and charged 50% of the scheduled service price.

  • If you arrive more than 15 minutes late but give us notice:

    • We may still accommodate you, but your service may need to be shortened to stay on schedule. The full price of the original service will still apply.

    • If you would like the full service despite being late, we may extend your appointment only if our schedule allows, and an additional time/service fee will be charged.

  • If you arrive more than 15 minutes late and we are unable to perform your service, it will be considered a late cancellation. A 50% fee of the scheduled service will be charged.

  • If we must reschedule due to lateness, a new deposit will be required to secure your future appointment.

Please note: The 50% late fee is separate from the new deposit and will not be applied toward your rescheduled service.

Consent Form Policy

  • All clients must complete a consent form prior to service.

  • Please arrive 15 minutes early or complete the form online in advance.

  • Services may be delayed or canceled if forms are incomplete.

Service Charge Policy

  • In accordance with Minnesota transparency laws (MN Stat. §325F.69), a 15% minimum service charge applies to returning clients whose previous appointment did not include gratuity.

  • This service charge is not a tip but ensures fair, consistent compensation for staff.

  • Guests are welcome to leave additional gratuity at their discretion.

Product Sales Policy

  • All product sales are final.

  • Due to hygiene and safety regulations, we cannot accept returns, exchanges, or issue refunds once a product has been purchased or removed from the premises.

  • Please double-check your selections before completing your purchase.

  • Our staff are happy to assist you with product recommendations before purchase.

Payments & Gift Cards

  • The remaining balance is due at the time of service.

  • To use a gift card for booking, please call 952-236-0265.

Service Package Policy

Except as otherwise required by law, the following Terms apply to all Service Packages purchased through Kōzē Head Spa & More (“Packages”).
By purchasing or using a Package, you acknowledge and agree to the following:

  • Package Location & Redemption

    • Packages are issued and redeemable only at Kōzē Head Spa & More.

    • Packages are non-transferable and cannot be shared, exchanged, or applied toward other services.

  • Payment & Validity

    • All Packages must be paid in full at purchase; payment plans are not available.

    • Packages do not expire and incur no maintenance fees.

  • Cancellations & Refunds

    • Within 30 days of purchase: You may cancel; services used will be charged at regular price, and the remaining balance will be refunded to your original payment method, minus a 10% transaction fee.

    • After 30 days: Packages are final and non-refundable.

  • Pricing & Service Changes

    • Prices, services, and package options may change without notice.

    • If a service in your package is discontinued, the remaining value may be applied toward another similar-value service.

  • Policy Compliance & Conduct

    • Clients must comply with all spa policies and safety standards.

    • Kōzē reserves the right to refuse service to anyone who violates policies, behaves inappropriately, or disregards health and safety guidelines.

  • Program Modifications

    • Kōzē Head Spa & More reserves the right to modify, suspend, or terminate package programs or terms without prior notice. Updated terms will be available in-spa or through official communication channels.
