Koze Head Spa & More - Booking Policy

Appointment Booking & Deposit:

  • Online Self-Booking: If clients book their appointments online themselves, the following non-refundable deposit is required at the time of booking:

    • $30 for services priced at $60 or more.

    • 50% of the service cost for services priced under or equal to $60.

    • $100 for all Permanent Makeup (PMU) services.

  • Phone Booking with Payment Link: If clients call to book an appointment, a payment link will be sent to them. This link will expire 30 minutes after it is generated. If the client uses this link to pay their non-refundable deposit, the deposit amount will be 50% of the total service cost, regardless of the service's original price.

  • This deposit will be applied towards the total cost of your scheduled service.

  • Appointments are not considered confirmed until the deposit has been received.

  • For group bookings of 2 or more clients, please contact us directly at 952-236-0265 to schedule your appointment and determine the required deposit amount. Group deposits will vary based on the services and number of clients.

  • Minors (under the age of 18) will need a guardian present to sign the waiver.

Booking for a Minor:

  • If you are booking an appointment for someone under the age of 18, a legal guardian must be present during the appointment to sign the necessary waiver.

Your Health & Safety - When to Reschedule or Postpone Your Appointment:

"Your health and the health of our staff are our top priorities. To ensure a safe and comfortable environment for everyone, please review the following guidelines:

Reschedule Your Appointment If You Are Experiencing Any of the Following:

  • Contagious Illness Symptoms:

    • Cold or flu-like symptoms (cough, shortness of breath, difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea).  

    • Fever (high temperature).  

    • Any other contagious illness symptoms, including skin rash, scabbing, or swollen lymph nodes.

  • Recent Exposure:

    • If you have been exposed to someone with cold or flu-like symptoms or a confirmed contagious illness within the last 5 days.

  • High-Risk Considerations:

    • If you are part of a high-risk or vulnerable population as defined by the CDC, please consider postponing your appointment.

Cancellation Policy:

  • We understand that unforeseen circumstances may arise. We kindly request that you provide at least 48 hours' notice if you need to cancel or reschedule your appointment.

  • Cancellations made with less than 48 hours' notice will result in a charge of 50% of the scheduled service price.

  • Cancellations must be made by calling 952-236-0265.

Consent Form Policy

  • All clients are required to complete a consent form prior to their appointment.

  • Please arrive 15 minutes early to fill out the form in person OR complete it ahead of time via the link sent to you by text.

  • To ensure your service starts on time, we kindly ask that all forms be submitted before your appointment whenever possible. Thank you for your cooperation!

No-Show Policy:

  • Failure to show up for your scheduled appointment without any prior notice will be considered a "no-show."

  • No-show appointments will result in a charge of 50% of the scheduled service price.

  • Future appointments may require a higher deposit or pre-payment.

Late Arrivals:

  • Please arrive on time for your appointment.

    • Arriving 15 minutes late without notice will be considered a no-show and will be charged 50% of the scheduled service.

    • If you arrive late but within the 15-minute window, we may need to shorten your service or reschedule.

    • If we perform a shortened service, the full price of the originally scheduled service still applies.

    • If we must reschedule due to your lateness, you will be charged 50% of the service.

    We appreciate your understanding and cooperation in respecting our time and scheduling.

Payment:

  • The remaining balance for your service is due at the time of your appointment.

Gift Card:

  • To book your appointment using a gift card, please contact us at 952-236-0265.
